Successful management of human factors in the workplace is an important contributor in minimising the risk of a major accident. Human Reliability Assessments can be carried out and their recommendations implemented, however how do we ensure the learning is shared and embedded into our ways of working? Unless we understand human behaviour and the many factors that can influence human performance and ensure that this understanding is embedded within our management systems and own behaviours, then standards can be compromised.
Often, the focus of Human Factors is at the workface, in those carrying out the tasks. However, it is the behaviour of those in leadership roles that are key to establishing a culture where we can learn from our own and others’ mistakes, without a fear of blame, and to ensure staff are competent, confident and able to complete safety critical tasks when required through strong, clear, procedures and training.
This presentation will refer to case studies where leadership and organisational culture have contributed to or failed to prevent major accident hazards, and draw on AXIOM’s experience of managing human factors on COMAH assets.
